Supports Planning

Total Care Services, Inc. provides Supports Planning services across the state of Maryland. We help you access Medicaid and non-Medicaid home and community-based services (HCBS) through the Maryland Department of Health, Office of Long Term Services and Supports. Our goal is to help you plan for and receive community services and supports that promote a safe and independent life in your home and in your community.

How to Apply

To receive services, you will need to meet certain eligibility requirements. The medical, financial, and technical criteria for eligibility vary for each program. What is the role of the Supports Planner?

TCS’ Supports Planners will help you identify, access, and coordinate services and supports to keep you healthy and safe. Your Supports Planner will:

  • Get to know you
  • Be your advocate
  • Provide information about services and supports in your community
  • Help you understand your options
  • Help you develop a plan of service
  • Help you realize your goals
  • Help you find providers
  • Make arrangements for delivery of services and supports
  • Monitor your services to make sure you are getting the support you need
  • Help resolve any issues that may occur

TCS’ Supports Planners are mobile and community-based. Because they live where you live, our Supports Planners are knowledgeable of the resources and supports available in your area.

Personal Assistance

Personal Assistance is the most common service used to support participants in their homes and in the community. Personal Assistance services improve your quality of life while maintaining your health and safety. Your Supports Planner will help you identify and coordinate Personal Assistance services to help with:

  • organizing and taking your medications
  • activities of daily living (ADLs)
  • instrumental activities of daily living (IADLs)
  • other needs integral to independent living


Medicaid’s HCBS programs are based on a philosophy of self-direction. This means you have the right to make choices that work best for you regardless of the nature or extent of your disability. As a Supports Planning Agency, our goal is to provide the information, resources, and support to empower you.

Self-direction means that you, or your representative, can make decisions about certain services. It also means you can manage your services with the help of a system of available supports. Deciding the role you wish to take is part of your person-centered planning process.

Self-direction also promotes choice and control over the delivery of waiver and state plan services, such as:

  • who provides your services
  • how you receive services
  • when and where they are provided
  • the rate of payment
  • hiring, managing, and firing attendants

TCS’ Supports Planners will help you create a person-centered plan that reflects your goals, needs, and choices. Our Supports Planners will help ensure that you understand your options and maximize your choice and control.
