Maryland - Supports Planning
Total Care Services, Inc. provides Supports Planning services across the state of Maryland. We help you access Medicaid and non-Medicaid home and community-based services (HCBS). These services are administered by the Maryland Department of Health, Office of Long Term Services and Supports. Our aim is to promote a safe and independent life in your home and in your community.
Who We Help
Our team of community-based Supports Planners serve people of all ages. We help people with complex medical needs, behavioral health needs, disabilities, and other challenges, who participate in the following programs:
- Community Personal Assistance Services (CPAS)
- Community First Choice (CFC)
- Home and Community-Based Options Waiver (HCBOW)
- Increased Community Services (ICS)
How to Apply
You will need to meet certain eligibility requirements to receive services. The medical, financial, and technical criteria for eligibility vary for each program. What is the role of the Supports Planner?
At TCS, we strive to empower people of different abilities to live and thrive in their own homes and community. Your Supports Planner help you identify, access, and coordinate services and supports to maintain your health, safety, and independence. They will also help you with waiver eligibility determination and maintenance. Your Supports Planner will:
- Get to know you
- Be your advocate
- Provide information about services and supports in your community
- Help you understand your options
- Help you develop a plan of service
- Help you realize your goals
- Help you find providers
- Make arrangements for delivery of services and supports
- Monitor your services to make sure you are getting the support you need
- Help resolve any issues that may occur
TCS’ Supports Planners are mobile and community-based. They live where you live. So, they are knowledgeable of resources and supports in your area.
Because we're community-based, we also understand the importance of safeguarding your privacy. Our Supports Planners use HIPAA-compliant technology to protect your health information.
Personal Assistance
Personal Assistance is the most common service used to support participants at home and in the community. Personal Assistance services keep you healthy and safe so you can enjoy a better quality of life. Your Supports Planner will help you identify and coordinate Personal Assistance services to help with:
- organizing and taking your medications
- activities of daily living (ADLs)
- instrumental activities of daily living (IADLs)
- other needs integral to independent living
ADLs are activities that are essential to living and taking care of yourself, such as:
IADLs focus on activities that promote independent living. Some of these activities include:
Person-Centered Approach
Medicaid’s HCBS programs are based on a philosophy that is person-centered. That is, you have the right to make choices that work best for you regardless of the nature or extent of your disability. As a Supports Planning Agency, our goal is to provide information, resources, and support to empower you.
Deciding the role you wish to take is part of your person-centered planning process. It means that you, or your representative, can make decisions about services.
A person-centered approach promotes choice and control over the delivery of waiver and state plan services. You choose:
- who provides your services
- how you receive services
- when and where they are provided
Your Supports Planner will help you create a person-centered plan that reflects your goals, needs, and choices. They will help ensure that you understand your options and maximize your choice and control.
