Total Care Services, Inc. (TCS) has been a provider of Targeted Case Management (TCM) through the Maryland Department of Health, Developmental Disabilities Administration (DDA) since 2014. TCS provides these services through its Coordination of Community Services program.

What is Coordination of Community Services?

TCS’ Coordination of Community Services program supports people of all ages with intellectual and developmental disabilities and their families. Our services are designed to help you learn about and gain access to resources, services, and supports in your community. Our goal is to empower you to make informed choices about your care and plan for your future.

 

Who do we serve?

Our Coordination of Community Services (CCS) program serves people across the state of Maryland. The DDA has four regional offices:

Southern Maryland Regional Office (SMRO)
Calvert, Charles, Montgomery, Prince George’s, and St. Mary’s Counties
Central Maryland Regional Office (CMRO)
Anne Arundel, Baltimore, Harford, and Howard Counties and Baltimore City
Eastern Shore Regional Office (ESRO)
Caroline, Cecil, Dorchester, Kent, Queen Anne's, Somerset, Talbot, Wicomico, Worcester Counties
Western Maryland Regional Office (WMRO)
Allegany, Carroll, Frederick, Garrett, and Washington Counties

 

How can TCS' Coordination of Community of Services Program benefit you?

Through our Coordination of Community Services program, we aim to build your independence. Our goal is to connect you with the resources and services you need to live a good life. What is a good life? It's different for each person. That's why we provide care coordination that is person-centered.

Whether your goal is to live on your own, find a job, participate in your community, or all that and more, we're here to support you. We support and encourage self-direction and self-sufficiency so you have more opportunities to live the life you want.

 

What is the role of your Coordinator of Community Services?

Making sure you understand all your health care options can be overwhelming. And navigating the systems built to deliver them can be even harder to figure out. Your CCS' role is to support you in your journey.

Your CCS will help you gain access to services, resources, and service delivery systems, like the DDA. They will link you to community resources and services that cover a wide range of needs and interests. Some examples are housing assistance, public transportation, medical services, or educational services. They'll also help you identify natural supports and explore recreational and social activities in the community that help you feel more connected.

Your Coordinator of Community Services (CCS) will partner with you to:

  • Identify your needs, goals, and preferences - what's important to you and for you
  • Inform you about important community resources and supports that are available
  • Empower you to make informed choices so you can live a full life in the community
  • Help you gain access to the DDA and other service delivery systems, services, and resources
  • Monitor your services to make sure they continue to meet your needs and wants

Our Coordination of Community Services program helps you plan for and receive support services in your community through one of three 1915(c) Medicaid home and community-based waiver programs including the Family Supports, Community Supports, and Community Pathways Waivers.
