Maryland – Supports Planning Program
What are Long Term Services and Supports (LTSS)?
Long Term Services and Supports are Medicaid and non-Medicaid home and community-based services provided to people of all ages with complex medical needs, behavioral health needs, and disabilities.
The Office of Long Term Services and Supports (LTSS) within the Maryland Department of Health (MDH) oversees the provision of services to participants in LTSS' programs, which include:
- Community Personal Assistance Services (CPAS)
- Community First Choice (CFC)
- Home and Community-Based Options Waiver (HCBOW)
- Increased Community Services (ICS) programs
Through our Supports Planning program, Total Care Services, Inc. (TCS) provides comprehensive case management and supports planning services across the state of Maryland. Our Supports Planning services help applicants and participants in LTSS’ programs plan for and receive their services.
Who should apply for LTSS' programs?
LTSS' programs are designed to empower older adults and people of all ages with disabilities and complex conditions to live safely in their own communities. The goal is to provide an alternative to institutional living for those who need specialized support in their homes. You may benefit from the home and community-based services and supports these programs provide if:
- You live in Maryland
- You have a chronic illness, medical condition, or disability
- You need support with activities of daily living
How is your eligibility for LTSS’ programs determined?
Your eligibility for LTSS' programs is based on certain financial, medical, and technical criteria, which vary by program. You will need to meet both the Medicaid Program's financial and medical requirements. Your Local Health Department will complete an assessment to determine your medical eligibility. Your local Department of Social Services or the Department of Health and Mental Hygiene will determine your financial eligibility.
